IMPORTANT FORMS, RULES & INFORMATION
Everything you need in one spot!
REQUIRED PARTY FORMS
As with any event, there are permissions and forms to be filled out before your senior can attend. Please be sure to download these required forms, follow the directions and turn them in as soon as possible. If you need assistance, please contact us directly.
ALLERGY EMERGENCY CARE PLAN
If your senior has a life-threatening allergy, please complete the Emergency Care Plan so that we know what is needed to ensure they are kept safe. Food providers have been asked to remove all nut products however foods are prepared in kitchens that may have nuts present and cross-contamination may have occurred. If your senior has a life-threatening allergy we can accommodate their brining any specially prepared food that is necessary. If epi-pens are part of their care regimen please let us know and plan on having your senior carry at least one epi-pen with them. A second epi-pen for a supervising chaperone is always appreciated if available.
The Party Rules
Here are the rules to ensure a safe and fun night for everyone! If you believe that your senior or you will have difficulty following these, then this just isn't the right opportunity for your senior. This after graduation party is not school or ASB affiliated, it is a private event hosted by senior parents and attendance requires adherence to the following rules.
01
Pre-Party Search
Seniors will be searched before boarding the bus at the beginning of the night.
02
100% Sober Party
No alcohol, drugs, or weapons are permitted. Because everything is provided, we ask seniors to not bring any bags, purses or personal items. However, we do encourage them to bring warm clothing and closed-toed shoes!
03
Completed Paperwork
Every senior must have a completed set of forms on file prior to the party. You can find the required forms located on this page.
04
Follow Rules

Willful disregard for, or failure to follow directions from chaperones, event staff/venue staff will result in the senior's removal. They will be placed in the custody of a parent or police. In the event that a senior needs to be removed from the party, their parent shall be responsible for picking them up from the party location.
