These forms are required for your senior(s) to attend the party. Please fill them out completely! Note the Life Threatening Emergency Care Plan form is only required if your child has a life threatening condition or allergy.<\/em><\/p>\n

\n\t\t6 RULES FOR THE PARTY\n\t<\/h2>\n\t

Six rules to ensure a safe and fun night for everyone!<\/p>\n

\n\t\t01\n\t<\/h3>\n

\n\t\tPre-Party Search\n\t<\/h3>\n\t

Seniors will be searched before boarding the bus at the beginning of the night.<\/p>\n

\n\t\t02\n\t<\/h3>\n

\n\t\t100% Sober Party\n\t<\/h3>\n\t

No alcohol, drugs, or weapons are permitted. Because everything is provided, we discourage bringing any bags, purses or personal items. However, we do encourage them to bring warm clothing.<\/p>\n

\n\t\t03\n\t<\/h3>\n

\n\t\tCompleted Paperwork\n\t<\/h3>\n\t

Every senior must have a completed set of forms on file prior to the party. You can find the required forms located on this page.<\/p>\n

\n\t\t04\n\t<\/h3>\n

\n\t\tFollow Rules\n\t<\/h3>\n\t

The chaperones and staff on-hand to ensure that everyone has a great time in a safe environment. Willful disregard for, or failure to follow directions from chaperones, event staff\/venue staff will result in the senior’s removal. They will be placed in the custody of a parent or police.<\/p>\n

\n\t\t05\n\t<\/h3>\n

\n\t\tParty Travel\n\t<\/h3>\n\t

Seniors may only travel to and from the venues on their assigned bus. Seniors will not be allowed to leave the party until its over or they’re picked up by a parent or guardian.<\/p>\n

\n\t\t06\n\t<\/h3>\n

\n\t\tMorning Pickup\n\t<\/h3>\n\t
